Our Advocacy Services will help patients:

OUR SERVICE MENU
Comprehensive Care Assessment
Everyone begins with a FREE Initial Consultation, lasting approximately 20-30 minutes. Here we will briefly discuss your situation and determine whether our services are appropriate for your specific needs.
Once the determination is made that we are a good fit and you are in agreement with next steps, we will then schedule your in-person Comprehensive Care Assessment for a 2 hour in-depth gathering of critical information and will discuss your situation/challenges in great detail along with expectations and goals.
From here we will develop a detailed plan of action. We will present you with your own personal Health and Wellness Care Plan tailored specifically to your needs and goals.
Professional Fee for this assessment is $300 paid in advance at time of scheduling. ***The cost of the assessment will be applied as a credit towards the first month of service when you sign a service agreement within 10 days of your delivered Care Plan***
LEVEL 1 Advocacy Services
Provide services to those with a new diagnosis, routine and non-urgent healthcare needs or just beginning their healthcare journey requiring less complex less frequent advocacy.
Most common situations for LEVEL 1 :
Already proactive in their care.
Newly diagnosed, needing guidance.
Caregiver becoming overwhelmed, needing help with decisions.
Need assistance with communicating and being heard.
Parent of a child worried they missed something, unsure of what to ask.
Needing assistance with establishing a relationship with a physician.
Seeking continuity of care while dealing with multiple specialists.
Basic medical bill/insurance review.

LEVEL 2 Advocacy Services
Provide services to those who are currently dealing with or are anticipating, extensive medical care. Likely to include a life-threatening diagnosis or serious chronic illness requiring in-depth advocacy.
Most common situations for LEVEL 2 :
Overwhelmed with long-term chronic illness, impacting everyday life.
Needing support with symptom and medication management.
Struggling with multiple providers unable to obtain accurate diagnosis.
Worried about long term consequences and prognosis.
Caregiver overwhelmed, no longer able to manage needing extensive assistance from outside resources.
Feeling unheard by healthcare team.
Unable to keep track of and organize medical records, insurance claims and current medications.
LEVEL 3 Advocacy Services
Provide services to those who are separated by distance. Trying to care, coordinate and monitor from out of the area causes serious concern for the health and wellbeing of your aging loved one.
Most common services for LEVEL 3 :
In-person once a month wellness assessment in the home, assisted living facility, or nursing home.
Review medication record, health status and report any concerns/recommendations monthly or bi-monthly to family/caregiver.
Assist and coordinate with family regarding sudden change in health status, emergency situation and/or hospitalization.
Act as liaison between out of area family and medical team until arrival of family or caregiver.
